Insulin Pump Consumables Assessment
If you would like your patients to be able to purchase Insulin Pump Consumables (IPCs) from the NDSS, you will need to complete an Insulin Pump Consumables Assessment Form to show they meet all relevant criteria. This needs to be signed by an endocrinologist, specialist physician or credentialed diabetes educator.
Continuous Glucose Monitoring Eligibility Assessment
The Australian Government is now providing access to fully subsidised continuous glucose monitoring (CGM) products through the NDSS. To be eligible, the person with diabetes will need to be assessed by an authorised health professional to determine if they meet certain specific criteria. Find out more about the specific criteria for each of the eligible groups.
If you would like people to be able to access CGM from the NDSS, you will need to complete and sign the Continuous Glucose Monitoring Eligibility Assessment Form specific to the individual to show that they meet all the relevant criteria. The health professionals authorised to certify CGM Eligibility Assessment Forms are endocrinologists, diabetologists, credentialled diabetes educators, or other health professionals specialising in diabetes (physicians, paediatricians or nurse practitioners).

If the individual needs to change their device, withdraw their access to CGM through the NDSS, or no longer meet the criteria, you will need to complete and certify a CGM ‘Update or Termination’ form, available below.
Product Order Forms
The NDSS gives your patients access to a large range of subsidised products that help them to affordably self-manage their diabetes. These products include:
- subsidised blood glucose testing strips
- subsidised urine testing strips
- free insulin syringes and pen needles (if you require insulin or an approved non-insulin injectable medication)
- subsidised insulin pump consumables (for approved persons with type 1 diabetes and gestational diabetes)
- continuous glucose monitoring products (for approved persons with type 1 diabetes and 'other' eligible conditions).
